A warm, anonymous check-in that helps teens understand what kind of support might help, and connects them to resources right in their community.


How it works

Three steps to finding support. It takes about a minute or two.

1. Check in anonymously

Answer nine short questions about how you’ve been feeling lately. No name, email, or ID required. There is no account to create.

2. See your support areas

The app maps your answers to broad support categories, like Stress Support or Social Connection, using clear, rule-based logic. No AI makes health decisions.

3. Connect with resources

You’ll see a small set of relevant community resources and self-guided supports, framed as “things that may help,” never as a diagnosis.


Designed to know as little as possible about you

No names. No accounts. No tracking.

No name, email, phone number, or ID is ever asked for or stored.

Staff see only group trends

Community-center staff never see an individual teen’s responses, only what a whole group reveals.

Small groups stay hidden

If fewer than ten teens submit in a group, that data is hidden entirely so no one can be identified.

Same safety resources for everyone

There is no safety-screening question. Every teen sees a crisis line, a trusted adult, and community support on their results screen, no matter how they answered.

For community centers

RiseWell is free for partner centers. Staff get a dashboard that shows only aggregate trends, such as how many teens checked in and which support areas are coming up most, and never anything about one individual. Centers can also add and verify their own local, free resources so the recommendations teens see are real and nearby.
